Puppy Basic Preventive Care Plan
- All needed vaccinations as determined by your AMC veterinarian
- Bi-annual Comprehensive physical exams, including:
- Otoscopic exam
- Ophthalmic exam
- Dental exam
- Rectal exam
- Neurologic evaluation
- Cardiovascular evaluation
- Weight/nutritional counseling
- Coat & skin evaluation
- Musculoskeletal evaluation
- Abdominal palpitation
- Urogenital evaluation
- Pulmonary/lung evaluation
- Heartworm test (1 per year)
- Three intestinal parasite fecal exams
- Four deworming (hookworms and roundworms)
- Ear swab and microscopic exam (1 per year)
- Free office exams during regular hours
- 15% off other services & medications (excluding outside service fees)
Puppy Basic Plus Preventive Care Plan:
- All Puppy Basic services
- Spay or neuter, including:
- Pre-anesthetic blood work (Blood cell count/differential exam of white blood cells/complete organ function screen)
- General anesthesia
- Anesthesia monitoring
- IV Fluids
- Pain medication injection
- Day hospitalization
Adult Dog Basic Preventive Care Plan:
- Heartworm test (1 per year)
- Urinalysis (1 per year)
- Intestinal parasite fecal exams (two per year)
- Ear swab and microscopic exam (first year only)
- Deworming (hookworms & roundworms) (two times per year)
- Free office exams during regular hours
- Blood cell count w/differential of white blood cells & internal organ function screen (one per year)
- 15% off other services & medication (excluding outside service fees)
- Yearly vaccinations (distemper parvo, leptospirosis, rabies, bordetella, vaccine boosters in first year)

Senior Dog Preventive Care Plan:
- Everything Dog Basic PCP Covers

- Annual X-Rays (up to three views)
